Immigration Physical

Individuals applying for citizenship, seeking Permanent Residency, or a Green Card are required to obtain an immigration physical prior to approval of their application. The U.S. Citizenship and Immigration Services (USCIS) makes it mandatory that immigration physicals are completed by a qualified USCIS Civil Surgeon so the exam is valid.

USCIS Designated Civil Surgeon

Dr. Jaddou is a Designated Civil Surgeon for  U.S. citizenship and Immigration Services. He has extensive experienced in performing the medical exam and filling out the I-693 form that needs to be submitted to the immigration office. All testing required by the U.S. Citizenship and Immigration Office is conveniently done at our offices. The necessary vaccines are available in our office for your convenience in the event you need them.  Somerset Family Medicine will assist you with the successful completion of the medical requirements of the application.

Convenient and Fast

  1. Immigration physical examinations are conducted six (6) days a week

  2. Appointments are available mornings, afternoons, evening, and Saturdays

  3. You can schedule an appointment online or by calling us

  4. If everything is complete and normal, we can give you the sealed envelope in 2-3 business days after your initial visit

Affordable

  1. $150 for both adults and children. This price includes, supplying and filling the I-693 form, TB skin test, all follow up visits, copy for you and a SEALED ENVELOPE for immigration and blood test (15 and older)

  2. If you need immunization, there will be additional charge. The doctor will discuss with you which immunization you will need, and the price of each.

Immigration Physical Frequently Asked Questions

  1. 1.Does insurance cover any part of the immigration physical such as shots etc?
  2. No, none of the immigration physical is covered by any insurance companies.  Shots are also not covered because this is not a preventative medical exam.  This service is related to immigration and has to do with filling out a special form, I-693 immigration form. We will give you a detailed receipt if you want to submit it to your insurance company.

  1. 2.What do I need to bring with me?

  2. Please bring identification such as driver license or passport.  If you have immunization records, bring them with you.  If you do not have the records, then it will be ok.  We will give you the necessary immunization to complete the application.

  1. 3.How do I make an appointment?

  2. Immigration Physical Exams are conducted Mondays through Saturdays.  You can schedule an appointment online or by calling us.
